JOB SUMMARY:
Under the general supervision of the Program Director/designee, functions as part of a team supporting consumers living in independent apartments in the community. Assists individuals in developing and maintaining viable living, working, and social situations by obtaining necessary services. Participates in crisis intervention services. Travels to consumers' residences to provide support and help maintain independence. Most duties are performed independently under general supervision.
ESSENTIAL TASKS:
- Visits consumers' residences to provide support as scheduled, helping modify attitudes and behaviors, and counseling consumers individually or in groups regarding service plans and resource mobilization.
- Gathers and evaluates information related to medical, psychological, and social factors, maintains treatment plans, and documents consumer progress. Reviews and follows up on service plans, assessments, and medical records.
- Maintains accurate case records, develops measurable service plans considering cultural and individual needs, and ensures plans include cultural, ethnic, and language considerations.
- Provides supportive counseling to assist consumers in modifying attitudes and behaviors.
- Develops and disseminates resource information in a clear manner.
- Uses cognitive-behavioral techniques to help meet service goals.
- Facilitates individualized services, explains available services, and instructs on daily living and social skills.
- Performs crisis assessments, refers consumers to community resources, and accompanies them to appointments as needed.
- Reports serious incidents immediately, completes incident reports, and manages consumer accountability and safety checks.
- Plans and escorts social or recreational activities, inspects consumer residences for safety, and reviews staff communication logs.
- Executes emergency plans, administers CPR or first aid, and assists with new personnel orientation.
- Adheres to attendance, infection control policies, and participates in meetings and training.
- Advocates for consumers, assists with vocational training placement, and collaborates with team members to address consumer needs.
- Performs other related duties as assigned.
MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:
Bachelor's degree plus two years of experience providing direct services to individuals with mental disabilities or relevant human service experience, or a bachelor's degree with a one-year practicum working with such individuals and one year of experience.
