Job summary

The Center at Arrowhead is seeking a dedicated care manager to join the patient care team in Glendale. You will help plan, develop and coordinate patient care in alignment with all applicable standards and regulations to ensure the highest quality care.

You will collaborate with administration, clinical staff and community agencies to facilitate smooth discharge planning, documentation, and ongoing care coordination. Experience in case management and strong HIPAA knowledge are essential.

Qualifications

• Minimum one year of case management experience.
• Must demonstrate leadership ability and work well with others.
• Must exercise good judgement and maintain highly confidential information.
• Must be knowledgeable about HIPAA compliance and requirements.
• Must be able to plan, organize, develop, implement and interpret department programs and goals.
• Must pass a pre-employment background check and drug screen; active fingerprint card required.

Responsibilities

• Meets with administration, medical and nursing staff to plan services with patients and families.
• Collaborates with patient, family, care team and social/health agencies for smooth discharge transitions.
• Ensures documentation is informative and follows policy and procedures.
• Assists with admission and discharge processes.
• Attends care plan meetings to provide insight and professional guidance.
