Job summary

Center at Arrowhead in Glendale, AZ seeks an experienced patient-care team member to support planning, development, and coordination of patient care per federal, state, and local standards. The role emphasizes high-quality, physician-driven care plans and collaborative problem-solving with families and care teams.

The position requires at least one year of case management experience, strong leadership, confidentiality, and HIPAA knowledge.

Qualifications

• One year of case management experience or more.
• Leadership ability and willingness to work harmoniously with other personnel.
• Good judgment and ability to maintain highly confidential information.
• Knowledge of HIPAA compliance and requirements.
• Ability to plan, organize, develop, implement and interpret department goals and objectives.
• Must pass a pre-employment background check and drug screen.
• Active fingerprint clearance card or equivalent.

Responsibilities

• Meet with administration, medical and nursing staff to plan services with patients and families.
• Coordinate with patient, family, care team, and social/community agencies for smooth discharge transitions.
• Ensure documentation is informative and aligns with Center policies and procedures.
• Assist with admission and discharge processes.
• Attend care plan meetings to provide professional guidance and patient-centered input.

Job Description

The primary purpose of this position is to be a member of a patient’s care team in order to assist in planning, developing and coordinating patient care in accordance with applicable federal, state and local standards, guidelines, regulations to assure that the highest degree of quality care can be maintained at all times.

Job Duties And Responsibilities

• Meets with administration, medical and nursing staff, as well as other related departments in planning services-to include patients and family
• Partners with the patient, family members, patient’s care team and Social , health and community agencies to insure a smooth transitions upon discharge from the Center
• Ensures that all documentation is informative, descriptive and follows the Center’s policy and procedures.
• Assists with the admission and discharge process.
• Attends care plan meeting to provide insight and professional guidance to patients needs

Experience And Specific Requirements

• A minimum one year case management experience
• Must possess leadership ability and the willingness to work harmoniously with other personnel
• Must possess good judgement and have the ability to maintain highly confidential information
• Must be knowledgeable of HIPAA compliance and requirements
• Must have the ability and to plan, organize, develop, implement and interpret the programs, goals and objectives of the department as it relates to the Company
• Must be able to pass a pre-employment background check/drug screen
