Job summary

Encompass Health in Glendale, AZ is seeking a seasoned Case Management Director to lead our Case Management department. You will oversee day-to-day operations, guide discharge planning, and champion patient and family advocacy while coordinating with care teams to optimize outcomes.

As a member of the leadership team, you will mentor Case Managers, manage regulatory compliance, and build partnerships with insurers and healthcare networks to support high-quality, compassionate post-acute care.

Qualifications

• CCM/ACMTM certification preferred.
• Active license if required for the discipline.
• Nursing track requires BSN and RN licensure.
• Three years hospital-based Case Management experience.
• Ability to work Weekdays and/or weekends, evenings and/or night shifts.

Responsibilities

• Oversee day-to-day operations of the Case Management department.
• Oversee interdisciplinary plan of care and discharge planning to optimize census and outcomes.
• Advocate for patients and families to ensure needs are met.
• Provide guidance and training on caseload management and regulatory procedures.
• Build relationships with insurers and healthcare networks.
