Job summary

The Center at Arrowhead is seeking a dedicated care manager to join the patient care team in Glendale. You will help plan, develop and coordinate patient care in alignment with all applicable standards and regulations to ensure the highest quality care.

You will collaborate with administration, clinical staff and community agencies to facilitate smooth discharge planning, documentation, and ongoing care coordination. Experience in case management and strong HIPAA knowledge are essential.

Qualifications

• Minimum one year of case management experience.
• Must demonstrate leadership ability and work well with others.
• Must exercise good judgement and maintain highly confidential information.
• Must be knowledgeable about HIPAA compliance and requirements.
• Must be able to plan, organize, develop, implement and interpret department programs and goals.
• Must pass a pre-employment background check and drug screen; active fingerprint card required.

Responsibilities

The primary purpose of this position is to be a member of a patient’s care team in order to assist in planning, developing and coordinating patient care in accordance with applicable federal, state and local standards, guidelines, and regulations to assure that the highest degree of quality care can be maintained at all times.

Responsibilities

• Meets with administration, medical and nursing staff, as well as other related departments in planning services‑to include patients and family
• Partners with the patient, family members, patient’s care team and Social , health and community agencies to insure a smooth transitions upon discharge from the Center
• Ensures that all documentation is informative, descriptive and follows the Center’s policy and procedures.
• Assists with the admission and discharge process.
• Attends care plan meeting to provide insight and professional guidance to patients needs

Qualifications

• A minimum one year case management experience
• Must possess leadership ability and the willingness to work harmoniously with other personnel
• Must possess good judgement and have the ability to maintain highly confidential information
• Must be knowledgeable of HIPAA compliance and requirements
• Must have the ability and to plan, organize, develop, implement and interpret the programs, goals and objectives of the department as it relates to the Company
• Must be able to pass a pre-employment background check/drug screen
• Active Fingerprint card
