Job summary

Odyssey House Louisiana, Inc. in New Orleans is seeking a full-time Case Manager to determine eligibility for guests to participate in the Agency's Low Barrier Shelter.

The Case Manager uses Motivational Interviewing to assess needs and create an effective individualized housing plan. The role involves screening, intake, assessment, and case management for guests, guiding them toward housing stability and connecting them with community resources.

Qualifications

• Bachelor's degree preferred.
• Excellent communication and organizational skills.
• At least two years of case management experience.
• Database management experience.
• Ability to work with homeless guests of diverse backgrounds and disabilities.

Responsibilities

• Provide case management to guests in the Low Barrier Shelter (LBS).
• Screen, intake, assessment, and housing plan development.
• Assist guests with obtaining vital records, IDs, and documentation.
• Assist guests with completing lease agreements and supporting documentation.
• Enter guests' information into agency databases and maintain records.
• Prepare reports and submit to the Project Manager in a timely manner.
• Maintain licensing requirements and oversee daily guest activities in the LBS.
• Refer guests to vocational, educational, housing and social services.
• Attend required meetings and trainings.
• Complete incident reports as required by policy.
