Job summary

Trinity Services, Inc. is seeking a Case Manager (formerly QIDP) in Illinois to lead person-centered planning and team coordination. You will manage plans, data, and outcomes while promoting empowerment and community integration for individuals supported.

The role includes supervising teams, coordinating health and financial needs, and maintaining QIDP status through annual CEU requirements. This on-site position emphasizes collaboration with the Network Director and care teams.

Qualifications

• Bachelor's degree in a Human Services field as defined by the Illinois Department of Human Services.
• At least one year of experience supporting individuals with intellectual/developmental disabilities.
• Must meet state and federal requirements for QIDP status and be approved by IDHS.
• Experience working with and guiding diverse support staff and individuals with varying abilities.

Responsibilities

• Guide and lead the Community Support Team in delivering individualized supports and services aligned with each person’s goals.
• Conduct Personal Outcome Measure interviews annually or as needed/requested.
• Ensure individuals' preferences and goals are reflected in person-centered documents.
• Facilitate Community Support Team meetings to develop Flourish Plans and address care decisions.
• Provide direct support to people in the home and community to help them achieve goals.
• Promote natural support networks to assist with relationships.
