Job summary

Friends of Switchpoint in Utah seeks a Case Manager Lead to guide day-to-day case management, provide training, problem‑solving support, and help with program procedures. The Lead offers subject‑matter expertise without formal supervisory authority; supervision remains with the Senior Manager/Case Management Supervisor.

This role strengthens consistency and teamwork across the Case Management team, supports resources navigation, and enhances client service delivery.

Qualifications

• Experience in case management, housing, homelessness services, or a related human-services field.
• Strong knowledge of community resources and available client services.
• Strong communication, organization, and problem-solving skills.
• Ability to provide peer support and training in a professional and collaborative manner.
• Ability to handle confidential and sensitive client information appropriately.
• Experience with HMIS or other case management/documentation systems preferred.
• Demonstrated ability to work independently while maintaining appropriate communication with leadership.

Responsibilities

• Maintain an assigned caseload and provide direct case management services.
• Complete required client assessments, service plans, case notes, referrals, follow-ups, and documentation.
• Assist clients with identifying and accessing housing, benefits, transportation, employment, identification, healthcare, and other community resources.
• Serve as a resource and point of contact for Case Managers who need guidance with client situations, resources, documentation, or program procedures.
• Provide support with complex or challenging cases and help Case Managers identify appropriate resources and next steps.
• Assist with maintaining consistency in case management practices, documentation, and client service expectations.
• Maintain an active caseload appropriate to the program, funding requirements, client acuity, and staffing structure.
