Job summary

Access Services is seeking a Case Manager of Justice Related Services to support individuals across Montgomery County with a focus on reentry and stability. The role requires driving across the county to meet clients in various settings and to coordinate linked services in collaboration with a multidisciplinary team.

Qualifications

• Bachelor’s degree in a related field such as psychology, social work, criminal justice is required.
• 2+ years of case management experience, preferably with adults with serious mental illness.
• Some criminal justice related experience preferred.
• Willingness to drive throughout Montgomery County during the workday to meet with clients is required.
• Background checks and driving history record will be checked.

Responsibilities

• Engage adults with serious mental illness to facilitate support and linkage to resources for successful reentry (housing, career, community involvement, etc.).
• Meet with clients in jail, in the community, at court, in hospitals, and through law enforcement.
• Organize and coordinate services for adults with serious mental illness in the forensic system using Blended Case Management guidelines.
• Ensure that the Access Services philosophy of care is carried out with each individual served and the service meets individual needs.
• Build collaboration through community outreach and awareness.
• Complete case notes and documentation related to the program.
• Attend regular team meetings and case reviews.
• Attend county, committee, and consumer meetings as needed.
