Job summary

Access Services is seeking a Case Manager of Justice Related Services to serve individuals across Montgomery County, with an office base in Plymouth Meeting, PA. You will guide justice-involved clients through housing, employment, and community resources, advocating for their reentry and wellbeing.

The role emphasizes dignity, mentorship, and collaborative teamwork, with a 40-hour workweek (Mon-Fri, 8AM-4PM) and on-call rotations. A valid driver's license and reliable vehicle are required.

Qualifications

• Bachelor's degree in psychology, social work, criminal justice, etc. is required.
• 2+ years of case management experience, preferably with adults with serious mental illness preferred.
• Some criminal justice related experience preferred.
• Willingness to drive throughout Montgomery County during the workday to meet with clients is required.

Responsibilities

• Engage adults with serious mental illness to facilitate support and linkage to resources for successful reentry (housing, career, community involvement).
• Meet with clients in jail, in the community, at court, in hospitals, and through law enforcement.
• Organize and coordinate services for adults with serious mental illness in the forensic system using Blended Case Management guidelines.
• Ensure Access Services philosophy of care is carried out with each individual served, and the service provided meets individual needs.
• Build collaboration through community outreach and awareness.
• Complete case notes and documentation related to the program.
• Attend regular team meetings and case reviews.
