Family & Children’s Services seeks a Jail Reentry Case Manager at the David L. Moss Criminal Justice Center to screen for risk, mental health, and substance use issues and identify diversion options to incarceration.
The role supports district court defendants, coordinates with attorneys and service providers, starts treatment assessments, and conducts jail re-entry planning, including group work for inmates.
Up to 34 Paid days off 1st year! The Jail Reentry case manager will provide risk, mental health and substance abuse screening services to district court defendants with the goal of identifying diversion alternatives to incarceration. Services are provided primarily at the David L. Moss Criminal Justice Center, however; some services may take place at other agency locations. Services are predominantly delivered to an adult population who carry a serious mental illness, substance abuse, and/or co-occurring diagnosis. The Case Manager will assist the District Attorney in managing community bound related cases in a timely and effective manner as the matters proceed through the court system. The Case Manager is responsible for establishing cooperation and communication among the participants, attorneys, treatment providers, and other agencies and professionals involved in each case. The Case Manager will provide assessments to start treatment and jail re-entry planning. Will provide group to inmates for substance use and thinking for change.
