Job summary

Family & Children's Services seeks a Case Manager to support adults with severe mental illness within a Certified Community Behavioral Health Clinic (CCBHC) framework. You will deliver essential services across income, housing, insurance, and education, with crisis intervention as needed and outreach in-person or via telehealth.

The role emphasizes person-centered care, data reporting, and collaboration with multidisciplinary teams in a community setting.

Qualifications

  • Case Manager II: high school grad or HS equivalence + 36 months behavioral health experience OR 60 college credits + 12 months behavioral health experience.
  • Case Manager III: Bachelor's Degree + 6 months behavioral health experience.
  • Case Manager IV: Bachelor's in social science; Case Management II cert or ODMHSAS course after hire.
  • Must have valid Oklahoma Driver License and acceptable driving record.

Responsibilities

  • Provide services for identified needs including income, insurance, housing, vocational/educational needs.
  • Educate individuals or groups on money management, symptom education, and interpersonal relationships; maintain household skills.
  • Provide crisis intervention and management; services range from monthly to weekly contact; in-person or telehealth.
  • Meet clients where they are in terms of mental health symptoms and treatment preferences.
  • Gather and complete all CCBHC data metrics.

About This Role
  • This position works with adults who have been diagnosed with a severe mental illness and/or co-occurring disorder.
  • Provide a variety of services for identified needs including but not limited to:
    • Income (employment, SSI/SSDI benefits)
    • Insurance
    • Housing
    • Vocational or educational needs
    • Access to reliable transportation
    • Food and clothing resources
    • Activities of Daily Living including personal grooming and hygiene
    • Substance use education and appropriate referrals
    • Informal supports and leisure activities
    • Crisis assessment and safety planning
    • Referrals to legal services
  • Educate (individual or group setting) a wide variety of skills related to topics such as: money management, symptom education and reduction, interpersonal relationships, maintaining a household, smoking cessation, etc.
  • Provide crisis intervention and management as needed.
  • Services range in intensity from monthly contact to weekly contact
  • Provide services in-person or via telehealth platforms
  • Meet clients where they are in terms of current mental health symptoms and treatment preferences
  • Gather and complete all CCBHC data metrics as needed.

Applicants will have previous background working in community mental health, inpatient psychiatric facilities, criminal justice, home health, or similar settings. Strong candidates with a passion for helping individuals with severe mental illness will also be considered.

Requirements
  • Case Manager II: Must be a high school graduate or have a HS equivalent + 36 months experience in behavioral health field OR 60 hours college credit in any field + 12 months of behavioral health experience required.
  • Case Manager III: Bachelor's Degree in any field + 6 months experience in a behavioral health field required
  • Case Manager IV: Bachelor's degree in social science field required Case Management II certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire.
  • Must have a strong commitment to the right and ability of each person served to live in normal community residences; and have access to helpful, adequate, competent, and continuous supports and services.
  • Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues and respect for client rights and personal preferences in treatment are essential.
  • Must possess a valid Oklahoma Driver License and satisfactory driving record and use personal automobile to travel to locations other than primary office.

  • Increased specialized services for veterans, those most in need, and those impacted by the opioid crisis.
