Job summary

North Range Behavioral Health in Greeley, CO is seeking a Jail-Based Behavioral Services (JBBS) case manager for the Adult Outpatient Program. This full-time role supports individuals in the Weld County Jail and continues services after release, focusing on coping skills, relapse prevention, and collaborative care.

You will document progress in electronic records, coordinate with therapists and community partners, and help clients access housing, employment, and other supports to promote

Qualifications

• Bachelor’s degree in a Human Services field.
• Valid driver’s license and an insurable driving record.
• Pre-employment screening including background check, fingerprinting, and drug and TB testing.
• Experience in behavioral or mental health services preferred.

Responsibilities

• Support clients in goal achievement and treatment plan progress.
• Coordinate and monitor care with therapists and staff.
• Refer and assist clients in accessing housing, employment, and services.
• Build partnerships with community organizations and manage related paperwork.
• Educate clients to develop coping skills and daily living strategies.
• Maintain progress notes and records to comply with requirements.
