Job summary

Lakeshore Talent in Colorado Springs is seeking a Case Manager IV to coordinate and monitor long-term care plans for individuals, families, and providers. This hybrid role combines field visits with remote coordination to promote independence, health, and safety.

You will develop person-centered service plans, advocate for clients, and document activities in electronic systems, while collaborating with the care team and community resources.

Qualifications

• Bachelor’s degree or 5 years of relevant experience in human services, case management, or working with vulnerable populations.
• Experience with Medicaid waivers or long-term care services preferred
• Strong communication, organization, and problem-solving skills
• Comfortable with technology, databases, and Microsoft 365

Pay: $23.46 – $24.36/hour

Schedule: Hybrid (field + remote) in the Colorado Springs area

Position Summary

The Case Manager IV supports individuals in accessing and coordinating long-term care services. This role works closely with individuals, families, and providers to develop person-centered service plans that promote independence, health, and safety.
