Job summary

PAM Health Specialty Hospital of Heritage Valley is seeking a Case Manager to coordinate care across disciplines for a designated patient population. You will review inpatient admission records to ensure appropriate level of care and help identify discharge needs.

Collaborate with physicians, therapists, social workers, and payors to optimize outcomes and resource use. A current BLS certification and state licensure are required, with 3–5 years inpatient experience preferred in acute settings.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include OT, PT, RN, RT, SLP, or social worker.
• Current BLS certification required.
• 3–5 years of inpatient experience in acute/IRF/LTACH settings.

Responsibilities

• Coordinate care across interdisciplinary teams for a designated patient population.
• Review inpatient admission records to ensure appropriate level of care.
• Assess discharge needs and coordinate discharge planning.
• Collaborate with payors and the care team to improve outcomes and resource use.
