AWAIC is hiring for a Case Manager – Integrated Health!

Are you a strong advocate, problem-solver, and relationship-builder who knows how to navigate complex healthcare and community systems? This might be the role for you.

The Case Manager-Integrated Health position has a special focus on helping survivors connect with healthcare, behavioral health, and substance use services. In this role, you’ll support participants as they navigate Medicaid, SSDI, primary and specialty care, mental health and substance use assessments, while coordinating with community providers and developing individualized plans that support health, safety, and independence.

We’re looking for someone who is compassionate, resourceful, organized, and persistent. Someone who can help make complicated systems feel a little more manageable and ensure participants don’t have to navigate them alone.
