Community Living Services (CLS) is a Michigan-based nonprofit organization focused on empowering individuals with intellectual and developmental disabilities to lead meaningful, self-directed lives. The Case Manager role advocates, coordinates services, and supports IPOS development to help people achieve independence and community inclusion.

Qualifications

• Bachelor's or Master’s degree required in human services or related field.
• Experience and ability to build trusting relationships with individuals, families, providers, and community partners.
• Detail oriented and able to complete paperwork timely.
• Independent role with travel to served sites and flexible scheduling.

Responsibilities

• Facilitate development and implementation of IPOS using person-centered planning principles.
• Coordinate, connect, and monitor services identified within the IPOS.
• Assist individuals and families in accessing Medicaid, SSI, Children's Waiver, Autism Benefit, and other entitlements.
• Support crisis planning and resolution.
• Coordinate health and safety strategies, including discharge planning.
• Participate in school transition planning and IEP meetings.
• Promote community inclusion, self-advocacy, and relationship building.
• Maintain accurate and timely documentation in compliance with agency, regulatory, and funding requirements.
• Participate in required trainings and professional development.
