Job description

Core Schedule:Schedule based on needs of clients & department.

Role Summary:
The Case Manager will be responsible for providing comprehensive services and support to young adults (ages 18-21) who have recommitted to the foster care system and are admitted into our independent living program. The role requires working closely with individuals to create a path toward self-sufficiency, including supporting educational, employment, financial, housing, and emotional needs. The Case Manager will serve as an advocate, counselor, and resource for each client, fostering growth, independence, and stability.

Essential Functions:
Develop and implement individualized service plans to assist clients in achieving independent living goals.

Monitor and assess client progress, ensuring they are meeting their goals and receiving necessary services.

Provide support and guidance on daily living skills such as budgeting, meal planning, time management, and decision-making.

Assist with securing stable housing, education, employment, and healthcare.

Facilitate referrals to community resources, including mental health services, substance abuse counseling, legal services, and life skills programs.

Coordinate with other social services agencies and community organizations to ensure that all client needs are met.

Advocate for the client's rights and needs, ensuring they are receiving appropriate services and support.

Conduct regular meetings with clients to assess progress, address challenges, and provide ongoing case management.

Provide crisis intervention when necessary and connect clients with emergency resources.

Maintain accurate records and documentation in compliance with program standards and confidentiality requirements.

Educate clients on available resources and ensure they understand how to access them.

Collaborate with foster parents, guardians, and other stakeholders to provide continuous care and support.

Assist clients in transitioning from the foster care system to full independence, including graduation from the independent living program.

Unplanned Activities: At BGH, we value teamwork. To meet departmental and organizational goals, the employee is expected to assist with activities that are not formal parts of the role’s essential functions.
Qualifications:

Education - A Bachelor’s degree in social services or related field.
  1. Experience/Credentials – Minimum of two years experience working with older adolescents and young adults, providing case management services working young adults and families in a child welfare or other social services organization. Use of Safe Crisis Management techniques and operation of company vehicles essential. Excellent verbal and written communication and organizational skills, including proficiency with appropriate computer software/hardware (i.e., Windows, Microsoft Excel, Outlook, Power Point, Word and other relevant software/hardware).
Other - Interact professionally with BGH employees, clients, visitors, government agencies and the general public. Maintain confidentiality of all client information. Must be at least 21 years of age and legally able to work in U.S. Valid driver’s license and insurability by BGH’s insurance carrier required. Successful completion of all background screenings and pre-employment drug screen. Successful negative, post-contingent offer and annual TB screenings.

Physical Demands: While performing the duties of this job, the employee is required to: stand, walk and sit; use hands to manually manipulate, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk and hear; taste or smell. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to read, write and converse in English.

Work Environment: The employee is exposed to a standard administrative office environment. The noise level in the work environment is usually moderate. S/he will also come into contact with BGH clients and others in clients’ homes, schools, in court and community meeting places. The employee works with youth and young adults who may exhibit challenging behaviors. As a result, employees may experience verbal aggression, physical aggression and may be at high risk of injury in the work environment, including but not limited to receiving: bites, cuts, kicks, punches, being spit on and the like. S/he will interact professionally with BGH employees, clients, visitors, volunteers (including Board members) government agencies and the general public.
