Job summary

People Incorporated Mental Health Services in Minneapolis, MN is seeking Case Manager II for our Waiver Case Management team in Hennepin County. You will work with clients diagnosed with chronic mental illness or other health conditions, developing person-centered Support Plans and authorizing services to promote independence and community integration.

This on-site role offers a hybrid schedule after training, with a focus on respectful communication, care coordination, and thorough

Qualifications

• Bachelor’s degree in social work, psychology, sociology, or related field.
• One year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
• Valid driver’s license with an excellent driving record and current auto insurance.

Responsibilities

• Attend bi-annual meetings with clients to monitor services.
• Develop and maintain collaborative relationships with clients with proactive, respectful communications.
• Assess client needs and ensure continuity of care.
• Maintain regular contact with collaborative partners.
• Make referrals and authorize services based on client needs and outcomes.
• Maintain records and document all services provided.

Case Manager II - Waiver Case Management (Hennepin County)People Incorporated is seeking Case Manager II's to join our Waiver Case Management- Hennepin County team to work with clients who are diagnosed with chronic mental illness or other ongoing health conditions. We connect people with services by developing person-centered Support Plans and authorizing services that help them live as independently as possible and integrated within their communities. Waiver Case Management involves more behind-the-scenes service coordination than traditional case management. Our Case Managers see clients twice annually and develop long term relationships by developing dynamic service plans. Our staff operate autonomously, creating their own hybrid schedules after completing initial training.
