Job summary

Central Counties Services in Temple, Texas, is seeking a motivated case management professional to maintain long-term contact with individuals, families, and service providers for medical, social, and educational needs. You will develop and implement recovery plans, coordinate with providers, and deliver related services in line with TRR and TAC guidelines.

The role requires disability-sensitive, trauma-informed practice, and strong documentation skills.

Qualifications

• Bachelor's degree in social sciences or related field.
• Experience in case management or related field.
• Knowledge of trauma-informed care practices preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Identify service gaps and coordinate with providers to address barriers.
• Develop and implement service and recovery plans for clients.
• Maintain and document case records per state and agency guidelines.
• Provide psychosocial rehabilitation training and related services as needed.
• Ensure confidentiality and timely documentation of records.
