Job summary

U.S.VETS Prescott, a leading nonprofit ending veteran homelessness, seeks an Experienced Case Manager II to support veterans facing housing instability and related challenges. You will provide trauma-informed, strengths-based case management and coordinate with the VA, landlords, and treatment providers.

Responsibilities include conducting assessments, developing individualized service plans, arranging housing, employment, and education services, and conducting regular client meetings and home

Qualifications

  • Bachelor’s degree in social work, psychology, counseling, or related field.
  • At least two years of work experience in case management or other roles in the social services arena.
  • Ability to pass background checks if assigned to programs involving incarcerated or recently released clients.
  • Strong communication, documentation, and organizational skills.
  • Proficiency in Microsoft Office and electronic records systems.
  • Valid and current driver’s license with clean driving record; ability to complete and pass a driver training course.
  • Experience working with homeless, veteran or justice-involved populations.
  • Certification in Substance Abuse Counseling (CCAPP/CAADE).
  • Experience facilitating groups or life skills classes.

  • Facilitate life skills groups and/or therapeutic groups for specialized veteran populations.
  • Assist with crisis intervention, urgent client matters, and after-hours support as needed.
  • Document all services and critical incident reports in a timely and professional fashion using MCR and other electronic client record systems.
  • Conduct random drug and alcohol screenings and ensure compliance with program rules and service plans.
  • Support reporting, audits, and file reviews in collaboration with the Program Coordinator and Director of Behavioral Health.

As aCase Manager II, you will provide trauma-informed, strengths-based case managementto veterans

  • Transitional Housing: Help veterans prepare for and transition into permanent housing.
  • SSVF (Supportive Services for Veteran Families): Deliver rapid rehousing and homelessness prevention services.
  • Permanent Housing (HUD-VASH, etc.): Focus on housing stabilization and long-term support.
  • Workforce Development: Assist clients with employment plans, resumes, and job readiness.
  • Justice-Involved Veterans: Provide pre-release and post-release support for veterans exiting incarceration.
  • Aftercare and Counseling Services: Offer ongoing case management and facilitate group or individual therapy (if licensed or certified).
