Job summary

Telecare Corporation is seeking a Case Manager II to provide primary case management and support for individuals with mental illness and co-occurring disorders, helping them live independently in the community. Under supervision, you will coordinate services, document progress, and assist clients in achieving their goals through community resources.

Full-time schedule from 8:00 AM to 5:00 PM, Monday to Friday, with compensation starting at $23.00 per hour.

Qualifications

• High School graduation or GED and 2 years of direct services in Mental Health or community services; or Associates degree and 1 year of experience; or Bachelor's degree in Social Services.
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use personal vehicle to drive clients to appointments and groups, etc.
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.
• Willingness to use your personal vehicle to attend meetings, etc.

What You Will Do to Change Lives

Schedule

Full-Time | 8:00 AM - 5:00 PM | Monday - Friday

Compensation

Expected starting wage range is $23.00.

Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services
