Job summary

Family Health Centers of San Diego is seeking a Case Manager II in El Cajon, CA. The role provides intensive therapeutic case management for medically underserved individuals under limited supervision, focusing on adherence and access to care.

The position requires a Bachelor’s degree in a related field and 3 years of relevant experience, with CPR credentials for AOD services. Travel between sites is occasionally required.

Qualifications

• 3 years of work experience providing human services to high-risk, medically underserved populations.
• Bachelor's degree in Social Science, Public Health, Healthcare Administration, or closely related field required.
• For AOD Services: CPR/BLS/ACLS/Advance Resuscitation Training required.

Responsibilities

• Completes required documentation accurately, timely and thoroughly.
• Conducts initial and ongoing assessments of client health and needs; sets level of client need.
• Creates formalized case plans and goals with clients and updates plans as needs change.
• Provides therapeutic interventions and intensive client support, including referrals and accompanying clients to services.
• Performs other duties as assigned.
