Job summary

Family Health Centers of San Diego is seeking a Case Manager I to provide community-based case management services for medically underserved populations. The role emphasizes developing individualized care plans, coordinating health and support services, and documenting progress in a timely manner.

Responsibilities include assessments, goal setting, and arranging internal and external referrals, with occasional travel between sites.

Qualifications

• Bachelor's degree in Social Science field or closely related area.
• Peer Support Specialist Certification is required.
• At least 1 year of experience providing human services to high-risk, medically underserved populations.

Responsibilities

• Complete all required documentation accurately and timely according to department standards.
• Assist in preparing reports as required.
• Conduct initial and ongoing assessment of clients' health and support service needs.
• Develop written care plans and goals with clients, including measurable targets.
• Provide direct client support, referrals, and accompany clients to appointments or services as needed.

Skills

Peer Support Specialist Certification

Bilingual English/Spanish

Valid California driver's license

Education
