Job summary

Qualifications

• High school diploma or GED required.
• Bachelor's degree in Counseling, Social Work or related field required (Case Manager II).
• Bachelor's degree in Counseling, Social Work or related field preferred (Case Manager I).
• Knowledge of community resources preferred.
• Bilingual preferred.
• Valid California driver's license required for staff who have regular driving duties.

Responsibilities

• Provide access, advocacy, resources and linkage to clients.
• Attend community events, provide outreach services and identify areas to strengthen.
• Administer program measurement tools and surveys to track data and outcomes.
• Interact productively within a multi-disciplinary team and other community organizations.
• Maintain record and tracking systems that meet agency standards.
