Job summary

Nashville Rescue Mission is actively seeking a full-time Case Manager to join our Men's Guest Services team. The primary responsibilities are to oversee and provide an ongoing short-term plan to help guests exit and gain independence from emergency services.

Qualifications

• Bachelor's degree from an accredited college or university with 1 year of relevant work experience; or Master's degree; or 10 years of relevant work experience and demonstrated ability to perform essential duties.

Responsibilities

• Conduct intake interviews assessing new and existing guest potential for NRM or other programs.
• Establish continuous contact with agencies and services providing assistance to guests.
• Establish appropriate guest plans and Covenant Agreements.
• Refer guests to and coordinate with internal/external resources to achieve goals.
• Assess progress and document progress and data in NRM database.
• Mobilize resources and interview as needed to achieve outcomes.
• Provide weekly updates on utilization of services.
• Undertake assignments from leadership prioritizing objectives.
• Regular attendance and punctuality are essential.
