Job summary

The State of Tennessee is seeking a professional Case Manager in the Department of Correction in Mountain City, TN. The role involves providing independent case management services to offenders in correctional facilities, coordinating services, and supporting supervision and rehabilitation objectives under applicable policies.

Responsibilities include offender assessments, developing and updating case plans, coordinating resources, and monitoring progress toward goals.

Qualifications

Education and experience: associate degree and one year relevant experience in corrections, supervision, case management, rehab services, or related field.
Substitution of experience for education allowed: full-time relevant experience may substitute for the degree.
Graduate coursework can substitute for one year of experience.

Responsibilities

Perform professional case management activities in correctional facilities with independence and accountability for assigned caseloads.
Review offender case information to support actions, supervision requirements, and program participation decisions.
Complete offender assessments, screenings, and case reviews to determine actions within authority.
Develop, implement, and update offender case plans and related documentation.
Coordinate services, referrals, and resources to support offender needs and reentry preparations.
Monitor offender status, behavior, and compliance indicators and initiate corrective actions.
