PIH Health, a nonprofit regional healthcare network serving millions in Los Angeles County and adjacent regions, seeks a Discharge Coordinator to support the Care Management team.

You will conduct advanced discharge coordination, engage patients and families, prepare regulatory discharge documentation, and coordinate post-acute care with real-time electronic tracking. This role acts as an operational extension of the RN Case Manager under delegated authority.

• Provide discharge coordination and clerical support for the Care Management team.
• Engage patients and families on forward-facing discharge processes.
• Prepare regulatory discharge documentation and post-acute coordination.
• Assist with real-time electronic tracking and timely discharge planning.
