Job summary

Behavioral Health Services at Redgate Memorial Recovery Center in Long Beach, CA seeks an experienced Case Manager II to provide comprehensive case management, recovery support, and care coordination for clients in its programs.

Under supervision, you will conduct screenings, develop care plans with clients, support housing and benefits navigation, and collaborate with the treatment team to promote ethical, evidence-based care. Strong documentation and crisis response skills are essential.

Qualifications

  • High school diploma or equivalent required; bachelor’s preferred for DMH program
  • Experience in substance use treatment or behavioral health setting is desirable
  • Knowledge of confidentiality requirements and DHS/HIPAA standards helpful

Responsibilities

  • Conduct comprehensive substance use screening, care planning, and treatment activities
  • Contribute to individualized care plans in collaboration with clients and treatment team
  • Support clients with relapse prevention, life and coping skills, discharge and aftercare planning, and recovery support
  • Provide case management services including benefit assistance, housing navigation, vocational support, and community reintegration.
  • Provide linkage and warm handoffs to community resources, including 12-Step and other recovery support groups.
  • Occasionally provide transportation for clients including DPSS and other linkages.
  • Build collaborative support systems with community employers, agencies, and housing providers.
  • Develop and maintain partnerships with governmental housing agencies to respond to client housing needs.
  • Coordinate with medical providers, social services, probation/parole, DCFS, courts, and other community partners.
  • Facilitate external communication for clients as approved by supervisor and as consented via Release of Information.
  • Prepare letters and progress reports for DCFS and other systems including required information for reporting as consented via Release of Information
  • May provide evidence-based services (e.g., CBT, MI, DBT, Seeking Safety) to assigned group and/or individual caseload
  • Maintain knowledge of evidence-based practices, medications for addiction treatment, co-occurring disorders, community resources, self-help support, and treatment philosophies
  • Monitor client progress and maintain accurate and timely treatment records and progress notes
  • May perform toxicology testing, take and record vitals as needed
  • May support with coaching, consultation, training, or development of new or entry-level staff
  • For residential cases, may make rounds and observe medication administration
  • Ensure compliance with regulatory and accreditation standards (DMH, DHCS, SAPC, HIPAA, 42 CFR Part 2, CARF)
  • Respond to behavioral crises and emergencies safely within scope
  • Document changes in client condition or behavior and manage emergencies
  • Participate in safety programming and maintain health and safety standards
  • Attend supervision, clinical meetings, trainings, and committees
  • Contribute to agency quality assurance and improvement initiatives

Job Details: Level: Experienced, Job Location: Redgate Memorial Recovery Center - Long Beach, CA, Position Type: Full Time, Salary Range: $26.41 - $29.05Hourly, Job Shift: Day, Statement of Purpose

Under the direct supervision of the Program Director/Supervisor, the Case Manager II provides case management, recovery support, and care coordination services to clients participating in assigned programs. The Case Manager II monitors, documents, and supports client progress, maintains ethical and evidence-based care, and participates in treatment team activities.

Major Tasks, Duties and Responsibilities
  • Conduct comprehensive substance use screening, care planning, and treatment activities
  • Contribute to individualized care plans in collaboration with clients and treatment team
  • Support clients with relapse prevention, life and coping skills, discharge and aftercare planning, and recovery support
  • Provide case management services including benefit assistance, housing navigation, vocational support, and community reintegration.
  • Provide linkage and warm handoffs to community resources, including 12-Step and other recovery support groups.
  • Occasionallyprovidetransportationforclients including DPSS and other linkages.
  • Build collaborative support systems with community employers, agencies, and organizations to assist clients in transitioning into employment.
  • Develop and maintain partnerships with governmental housing agencies to respond to client housing needs.

  • Maintain general knowledge of evidence-based practices, medications for addiction treatment, co-occurring disorders, community resources, self-help support, and treatment philosophies
  • Monitor client progress and maintain accurate and timely treatment records and progress notes (e.g., daily activities, service entries, intake and discharge documentation, etc.)
  • May perform toxicology testing, take and record vitals as needed, chart relevant information
  • May support with coaching, consultation, training, or development of new or entry-level staff within their scope of practice.
  • For Case Managers assigned to residential programs, may make rounds every 30 minutes for 72 hours on withdrawal management clients, document and record withdrawal symptoms.
  • For Case Managers assigned to residential programs, may observe and document client self‑ administration of medications.
  • Ensure compliance with regulatory and accreditation standards, including but not limited to DMH, DHCS, SAPC, HIPAA, 42 CFR Part 2, CARF, etc.
  • Respond to behavioral crises, medical emergencies, suicidal ideation, overdose including administration of anti-overdose medications as needed, or acute intoxication safely, within scope of practice
  • Report and record any changes observed in client condition or behavior, including unusual incidents. Identify, report, and assist with the management of emergencies.
  • Participate in safety programming to promote client and staff safety. Maintain health and safety standards of facility.
  • Attend supervision, clinical meetings, and continuing education to maintain credentials.
  • Participate in treatment team meetings, case conferences, and program planning.
  • Contribute to agency quality assurance and improvement initiatives.
  • Maintain professional boundaries and standards. Treat clients with respect, care, and concern. Comply with confidentiality requirements at all times.
  • Maintain familiarity with BHS policies and procedures and other applicable regulations; perform job duties accordingly. Represent BHS well in all contacts.
  • Courteously answer phones, appropriately directing calls and taking messages. Facilitate client external communication as approved by supervisor.
  • Attend meetings and trainings and participate in committees as assigned.

Case Manager II Competencies and Performance Expectations:

  • If assigned to SUD program, ability to demonstrate familiarity with and practice of 12 core competencies for SUD counselors
  • Ability to effectively document within required note structures (e.g., SOAP, GIRP, etc.)
  • Ability to recognize the symptoms of and appropriately treat substance use and co-occurring disorders (i.e., mental health, medical health, etc.) within the scope of practice.
  • Ability to identify and respond appropriately to a crisis within scope of practice including but not limited to overdose, behavioral challenges, contraband protocol, emergency codes, etc.
  • Ability to screen and/or assess for acute intoxication and withdrawal symptoms within scope of practice
  • Ability to recognize personal biases within diverse populations. Maintain cultural sensitivity at all times.
  • Ability to adhere to professional standards
Prerequisite Qualifications

The following are the qualifications required to perform the essential functions of this position. Qualifications may be subject to modification based on the Americans with Disabilities Act.

If assigned to an SUD program, minimum of high school diploma or equivalency. Certified by a state-recognized substance use counselor certification body, with at least 2 years of substance abuse experience. Maintain certification in good standing.

If assigned to an DMH program, a bachelor's degree in social work or related field, preferred. AA degree plus related experience in lieu of degree.

Obtain NPI number, EMR system enrollment within 30 days of employment.

Ability to operate required computer programs for position, including but not limited to Microsoft Office Suite, electronic timekeeping system, electronic learning management system, upon hire or demonstrated competency developed within 90 days of employment.

Must have valid California driver’s license and liability insurance if driving a personal or company vehicle on BHS business.

Vision, hearing, manual dexterity, and eye-hand coordination must be adequate for the performance of job duties. Able to sit at a desk, use a keyboard, write and physically perform other job duties. Able to move about the facility to observe clients and staff. Complete annual TB, maintain good standing.
