Job summary

The ideal candidate will hold a bachelor's degree in social work or equivalent experience, strong communication and organizational skills, and be ready to complete CTI training within 90 days. Travel within the service area may be required.

Responsibilities

  • Provides outreach to community partners and represents VOA values in interactions to recruit landlords and support program participants.
  • Advocates for participants, coordinating with agencies and service providers to access health care and daily living services.
  • Demonstrates good clinical judgment and culturally appropriate engagement with participants and families.
  • Completes required documentation and HMIS/EHR entries within set timelines.

JOB SUMMARY AND QUALIFICATIONS

The Case Manager’s primary duty is to provide case management to the families who are residing in the Family Emergency Shelter. The Case Manager will assist in ensuring effective daily operations of the shelter as well.

WHAT YOU MUST HAVE:
  • Bachelor's degree in social work or a related field or a least five (5) years of experience in the field and no degree would qualify of this position.
  • Requires a demonstrated ability to provide a reasonable skills: family needs assessment, knowledge, and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers.
  • Must have a commitment to the principles of the project.
  • Excellent verbal and written communication skills, high degree of organization, and creative problem- solving skills.
  • Must have a personal automobile, valid driver's license, liability insurance, and be willing and able to travel within the full-service area.
  • Must complete required case management and (Critical Time Intervention) CTI training within 90 days of hire.

RESPONSIBILITIES
  1. Provides outreach to community partners and represents VOA values in interactions with community partners to increase awareness of and recruit landlords to work with our client families in the program a) Perform assessment and developing/monitoring case plans andconducting necessary follow-up activities b) Establish linkages with appropriate agencies and serviceproviders in the area/community c) Provide referrals and coordinating with community partnersand other service providers to assist participants in accessinghealth care services and daily living services d) Decide how resources are allocated to participants on the basisof need and eligibility, according to grant guidelines e) Educate participants on issues, such as supportive servicesavailability and participant rights f) Provide assistance to participants in obtaining public benefits g) Provide assistance to participants in obtaining a birth certificate, driver’s license, or/and TARC tickets h) Provide/refer for supportive services to participants, includingpersonal financial planning services, transportation services, income support services, recovery programs, legal services, and housing counseling for the purpose of overcoming obstaclespreventing obtaining stable housing and or emergency shelter. i) Complete required documentation (including progress notes)within 48 hours of contact Complete and submit the required documentation for entryinto the Homeless Management Information System (HMIS and Electronic Health Record system within 48 hours of entryand service to Program Support Coordinator and Portal billing logs to the Program Manager weekly. k) Demonstrate good clinical judgment in decision makingregarding participants l) Demonstrate ability to relate to participants and their families in a culturally appropriate manner. m) Performance Quality Improvement (PQI) duties as assigned bysupervision & PQI Committee n) Facilitate the Critical Time Intervention (CTI) evidence-basedmodel to program participants
  2. Advocate for the participant, integrating cultural values into their case plan.
  3. Identify systemic barriers and communicates with organizationalleadership about these barriers to work collaboratively to find viablesolutions.
  4. Comply with all policies and procedures of the program and the Councilon Accreditation, Office of Resilience and Community Services, andVOA Mid Policies and Procedures.
