Department: TPC-Prairie Outpatient and Health Homes Site.

This position is eligible for a $2500 retention bonus.

Job Summary:

The Providence Center Case Manager ICT Prairie Team will coordinate services for consumers to minimize stressors and facilitate the reduction of psychiatric and substance abuse disorder symptoms. The goal is to encourage consumers to attain the highest possible levels of independence.

Duties and Responsibilities:

  1. Coordinate a variety of services.
  2. Provide support counseling, problem solving, contracts, and limit setting.
  3. Establish attainable goals with the consumer.
  4. Participate as a member of a multidisciplinary team in developing and implementing therapeutic services, including working with substance abusers, as it relates to case management.
  5. Make assessments and provide appropriate crisis interventions and risk management as needed.
  6. Teach consumers adult daily living skills.
  7. Maintain a clean, healthy, and safe environment at all times.
  8. Initiate treatment and advocate for consumers within the network of care.
  9. Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
  10. Transport consumers in own vehicle or Center van to planned activities and appointments.
  11. Complete recordkeeping in accordance with Health Information Services (HIS) and other center requirements within required time frames and abide by center confidentiality policy.
  12. Maintain cooperative relationships with Center staff, consumers, community agencies, and the public.
  13. Attend trainings, case presentations, and conferences.
  14. Perform other duties as assigned.

Requirements:

  1. Associate's degree required; Bachelor's degree in Human Services or related fields preferred.
  2. Experience in Human Services field preferred.
  3. Ability to read and interpret documents such as policy and procedure manuals. Ability to write routine reports and correspondence.
  4. Effective speaking skills before team members and other employees; ability to work with community agencies.
  5. Valid driver's license and insured auto required.
  6. Flexibility in work schedule and duties.
