Job summary

Interfaith Community Services in Escondido, CA seeks a Homeless Outreach Specialist 1 to provide field-based outreach, crisis intervention, and short-term case management for individuals experiencing homelessness with SUD/SMI. You will connect clients to housing, health, and supportive services while coordinating with clinicians, outreach workers, and community partners.

The role requires a high school diploma with preferred bilingual ability (English/Spanish) and experience in trauma-informed

Qualifications

• High School Diploma, required
• Bachelor’s degree in related social services or equiv years of relevant work experience, preferred
• Experience working with severely mentally ill, medically fragile, and homeless population, preferred
• Bilingual English/Spanish, preferred

Responsibilities

• Conducts outreach and engagement with individuals experiencing homelessness
• Identifies, engages and links underserved populations with SUD and behavioral health services
• Provides direct services to homeless clients and families, including intake, assessment, referrals and case management as well as client advocacy
• Enter data into HMIS/CES and other electronic data systems as required by funders and Interfaith
