Job summary

VOA Northern California & Northern Nevada is seeking a Case Manager I for the Singles Program at the Mather Community Campus. The role involves case management, assessments, counseling, and coordinating resources for participants.

A bachelor’s degree in social work and experience in human services are preferred. Responsibilities include developing service plans, conducting sessions on housing and life skills, and maintaining thorough documentation to ensure compliance with CARF standards.

Qualifications

• Bachelor’s degree in social work or related field preferred with two years of experience in human services.
• Knowledge of CPS/APS and services for homelessness.
• Experience with strength-based case management and trauma-informed care.

Responsibilities

• Perform case management with participants and develop individualized service plans with housing focus.
• Review case plans weekly and adjust goals as needed.
• Conduct group and individual sessions on life skills, employment, housing, and finances.
• Document all contact, screenings, service plans, and reporting records.
• Coordinate referrals with community agencies and provide crisis intervention as needed.
• Maintain client files and ensure compliance with reporting standards.
