Job summary

Old-Pueblo-Community-Services in Tucson, AZ seeks a Case Manager I to engage, assess, support and coordinate care for people experiencing homelessness and related conditions. Responsibilities include intake, recovery-plan updates, and life-skills sessions within a HIPAA-compliant EHR system.

Requirements include an Associate, Bachelor’s, or Master’s degree, CPR/First Aid certification, and willingness to use evidence-based practices.

Qualifications

• Must have an Associate, Bachelor’s or Master’s degree.
• CPR/First Aid certification or ability to obtain it.
• Basic computer skills (Word, Excel, email).
• Willingness to learn evidence-based practices (CPI, MI, Housing First, Harm Reduction).
• Self-starter, team player, able to meet deadlines and adapt hours.

Responsibilities

• Complete intake process with new clients.
• Deliver services aligned with recovery plans to help clients reach goals.
• Update recovery plans as services are completed.
• Complete C-SSRS and coordinate risk plans with supervisors.
• Connect clients to mainstream services and other organizations.
• Provide case management and life skills sessions.
• Co-facilitate life skills groups with supervisors.
• Facilitate ART meetings when needed under guidance.
• Document services in the EHR and maintain files per HIPAA.
• Ensure program guidelines are followed and address non-compliance.
• Collaborate with OPCS team and external partners.
