Job Summary and Qualifications

The Moral Injury and Preventing Suicide program assesses and attempts to stabilize Veterans who are experiencing suicidal ideation or at risk of suicidality. The Case Manager is a peer support, also working to train community partners on suicide awareness and prevention. This position requires a Bachelor of Social Work or a related undergraduate degree with less than five (5) years of related work experience; a person with five (5) years of experience in the field and no degree; or a veteran with three (3) years of experience in the field.

  • Ability to talk and openly engage in discussion on suicidal ideation and harm reduction resources.
  • Demonstrated ability to provide a reasonable combination of the following knowledge, abilities, and skills: family needs assessment, knowledge, and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers.
  • Commitment to the principles of the project. Veteran of the United States Armed Forces preferred. Excellent verbal and written communication skills, high degree of organization, and creative problem‑solving skills.
  • Personal automobile, valid driver’s license, liability insurance, and willingness and ability to travel between the counties we serve up to 70% of the time.

Responsibilities
  1. Provide outreach to community to increase awareness of suicidality.
  2. Lead group sessions and community training classes.
    • Perform assessment and developing/monitoring case plans and conducting necessary follow‑up activities.
    • Establish linkages with appropriate agencies and service providers in the area/community.
    • Provide referrals and coordinate with VAMCs and other service providers to provide health care services and daily living services to participants.
    • Decide how resources are allocated to participants on the basis of need.
    • Educate participants on issues, such as supportive services availability and participant rights.
    • Assist participants in obtaining VA benefits.
    • Assist participants in obtaining and coordinating other public benefits.
    • Provide/refer for supportive services to participants, including personal financial planning services, transportation services, income support services, fiduciary and representative payee services, legal services, childcare, and housing counseling.
    • Demonstrate good clinical judgment in decision making regarding participants.
    • Demonstrate ability to relate to Veterans and their families in a culturally appropriate manner.
    • Performance Quality Improvement (PQI) duties as assigned by supervision & PQI Committee.
  3. Advocate for the Veteran, integrating their needs/wants into their case plan.
  4. Actively participate in agency committees, projects, and initiatives as required.
  5. Assist with quarterly Case Record Review, as a reviewer, as needed.
  6. Comply with all policies and procedures of the program and the Council on Accreditation.
