Job summary

GMHC is seeking a Housing Substance Use Case Manager (CASAC) in New York City to provide client-centered counseling and case management within the HUD Connection to Housing & Care Program. You will conduct intakes, treatment planning, and ongoing support to promote housing stability and recovery while coordinating with medical, mental health, and housing services.

The role requires a CASAC license and experience delivering counseling in community or healthcare settings.

Qualifications

• Experience providing substance use counseling and case management.

Responsibilities

• Manage caseload of up to 25 clients across NYC.
