S:US is seeking a Case Manager for their men’s shelter in Brooklyn, NY. This role requires strong case management, documentation, and coordination of housing services for chronically homeless individuals.

Qualifications

• Bachelor’s Degree in Human Services preferred; HS diploma or GED required.
• Experience working with chronically homeless individuals with co-occurring disorders.
• Bilingual preferred; NYS Driver’s License preferred but not required.

Responsibilities

• Conduct psychosocial, vocational/educational, and housing assessments with documentation.
• Document provision of supportive social services and client progression toward housing goals.
• Prepare housing applications and assist clients with the housing application/interview process.
• Provide case management to an assigned caseload and develop service plans.
• Refer clients to in-house and community medical, mental health, and social services.
