Job summary

Qualifications

• Bachelor's degree with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field with relevant experience.
• Experience with adults experiencing serious mental illness preferred or required depending on degree.

Responsibilities

• Process client intakes, admissions and discharges.
• Assess mental status, medical/dental health, substance use, education and employment, and housing needs.
• Maintain a caseload and provide biopsychosocial assessments, recovery planning, case management and housing supports.
• Develop comprehensive service plans and coordinate housing referrals and community connections.
• Coordinate in-house and/or community appointments (medical, dental, therapy).
• Make and receive calls regarding client issues and maintain contacts with service providers.
• Develop and monitor safety plans.
• Maintain documentation for compliance and ongoing client needs.
• Operate agency vehicle for client transport as needed.
