Job summary

Jericho Project is seeking a Case Manager for Logan Fountain (Brooklyn), a 105-unit Permanent Supportive Housing program. You will provide case management for adults with mental health and substance use histories, coordinating with clinicians and the workforce team to support housing stability.

Responsibilities include assessments, service planning, data accuracy, benefits referrals, and community integration. Weekend work and bilingual skills are preferred.

Qualifications

• Bachelor's degree in human services or related field required.
• Experience with homelessness, substance use, mental health and/or trauma settings preferred.
• Willing to work a weekend day.
• Strong organizational, writing, communication, and computer skills.
• Candidates must be self-starters with the energy to work with high-need individuals.
• Bilingual English/Spanish strongly preferred.

Responsibilities

• Maintain a case load of high risk/high intensity tenants.
• Provide enhanced case management using Housing First approaches.
• Conduct assessments and program intakes for residents.
• Ensure data accuracy and complete documentation.
• Develop and update person-centered service plans (every 6 months).
• Coordinate with psychiatry and manage appointments.
• Provide referrals to public benefits and linkages.
• Identify residents’ strengths and needs and support community integration.
• Coordinate aftercare services and follow-up contacts.
