Job summary

NAICA is seeking a Case Manager in the New York City area to manage a client caseload and deliver on-site, person-centered services. The role involves intake, case management, and aftercare, aligned with NAICA’s values and mission to empower clients within the local community.

The Case Manager will conduct initial assessments, coordinate ILPs with other providers, and document progress while ensuring confidentiality and respectful collaboration with diverse teams.

Qualifications

• Bachelor’s degree in social work, psychology, or related field required.
• Strong written and verbal communication skills.
• Bilingualism preferred.
• Ability to maintain confidentiality.
• Ability to collaborate with diverse groups.

Responsibilities

• Promptly conducting clients’ initial intake assessments and addressing critical needs.
• Completing bio-psycho-social assessments within 30 days of intake.
• Developing Independent Living Plans (ILPs) with other providers.
• Maintaining thorough client records and reporting incidents to supervisors.
• Connecting clients with housing, employment, and public assistance resources.
