BASIC PURPOSE

Partner with families of children 0-5 as they tackle life's stressors by home visiting through the Healthy Families America model.

ESSENTIAL DUTIES & RESPONSIBILITIES

• Provide direct services to individuals and families referred to the Healthy Families program; in accordance with Healthy Families America’s Best Practice Standards; maintaining a reliable level of attendance necessary to perform the job; scheduling to be based on what best meets the needs of our participants and Family & Children’s Center.
• Interview and assess family history and strengths of those families newly referred to Healthy Families Program.
• Adequately plan and prepare for each weekly visit with referred and enrolled families.
• Record visits and activities, within 48 hours, in the EHR system.
• Attend and participate in staff meetings.
• Review case updates with clinical supervisor.
• Complete 40 hours per year staff development (20 for part/time) including a diversity credit, Blood borne Pathogen credit, wellness/self‑care credit, CMS (billing fraud) training, abuse/neglect indicators & reporting requirements and continuing education in topics directly related to this position.

Qualifications

Education, Training & Work Experience: Bachelor’s degree in related field with minimum of three years’ relevant experience, preferred.
