Job summary

The Family & Children's Center in Wisconsin seeks a Case Manager for Healthy Families in Viroqua. You will partner with families of young children, provide home visiting services, and support families through case management.

A bachelor's degree and at least three years of relevant experience are preferred. Responsibilities include home visits, documenting activities, attending staff meetings, and pursuing ongoing training to meet program standards.

Qualifications

• Bachelor’s degree in related field with minimum of three years’ relevant experience.
• Knowledge of child development and family systems, with strong communication skills.
• Experience using computer programs like Microsoft Office, DAISEY and INSYNC.

Responsibilities

• Provide direct services to families referred to Healthy Families.
• Interview and assess family history and strengths of new referrals.
• Plan and prepare for weekly home visits.
• Record visits within 48 hours in the EHR system.
• Attend staff meetings and participate in supervision.
• Complete 40 hours per year of staff development and training.
