Job summary

New Hope Treatment Centers is seeking a Case Manager to coordinate individualized services for residents in our Beaver, WV program. The role ensures treatment goals are met, care is continuous, and all regulatory requirements are followed.

You will work with therapists, medical staff, families, and external agencies to secure needed services, document progress, and maintain clear communications with the interdisciplinary team.

Qualifications

  • Full-time case management role supporting residents in behavioral health.
  • Must meet QMHP requirements where applicable and have a qualifying degree.

Responsibilities

  • Coordinate and manage individualized service plans for residents.
  • Monitor progress, conduct reviews, and communicate updates to the team.
  • Collaborate with therapists, medical providers, families, and community resources.
  • Advocate for residents to access clinical, educational, medical, and social services.
  • Maintain accurate documentation compliant with regulations and Medicaid requirements.
  • Facilitate treatment team meetings and discharge planning.
  • Maintain confidentiality and HIPAA compliance.
  • Attend weekly team meetings and coordinate with referrals and funding sources.
  • Strengthen family and kinship relationships through visits and planning.

Job Details: Job Location: NHWVII - Beaver - Beaver, WV 25813, Since our opening in 1987, New Hope Treatment Centers has been a welcoming place for young people in moments of crisis. Our programs have played a role in countless success stories, thanks to our relational approach to behavioral healthcare. The Case Manager is responsible for coordinating, monitoring, and documenting individualized services for assigned residents to support treatment goals, continuity of care, and regulatory compliance.

Essential Duties and Responsibilities

The essential functions include, but are not limited to, the following:

  • Coordinate and manage individualized service plans for assigned residents in accordance with clinical, organizational, and regulatory requirements.
  • Monitor resident progress, conduct routine reviews, and communicate updates to the interdisciplinary treatment team.
  • Collaborate with therapists, medical providers, families, external agencies, and community resources to ensure continuity of care.
  • Advocate for residents by facilitating access to appropriate clinical, educational, medical, and social services.
  • Maintain accurate, complete, and timely documentation in compliance with applicable state regulations, licensing standards, Medicaid requirements (if applicable), and organizational policies.
  • Facilitate treatment team meetings, discharge planning, and service coordination activities.
  • Maintain professional boundaries and adhere to confidentiality requirements, including HIPAA regulations.
  • Meets weekly during scheduled team meetings with non-LCS staff to discuss resident needs, appointments and progress.
  • Maintains regular contact with families, legal guardians, representatives of referral agencies, and courts (as required); provides regular updates on resident progress and ensures that residents, families, legal guardians and representatives of referral agencies are included in all aspects of planning for the residents.
  • Provides resident progress information to funding sources in a manner and timelines required by the funding source.
  • Facilitate and coordinate visitations and work to strengthen family and kinship relationship
  • Perform other duties as assigned consistent with the nature of the position.

  • Assessment & Care Coordination – Ability to coordinate, monitor, and evaluate individualized service plans based on clinical and psychosocial needs.
  • Documentation & Regulatory Compliance – Demonstrates accuracy, timeliness, and completeness of documentation consistent with organizational policy and regulatory standards.
  • Interdisciplinary Collaboration – Effectively communicates and collaborates with clinical staff, families, and external stakeholders to support treatment goals.
  • Patient/Resident Advocacy – Identifies service needs and facilitates appropriate internal and external resources.
  • Confidentiality & Ethical Practice – Adheres to HIPAA, privacy, and professional ethical standards.
  • Crisis Awareness & Response – Recognizes behavioral health risk indicators and responds appropriately within scope of role and training.
  • Cultural & Trauma-Informed Care – Applies principles of cultural sensitivity and trauma-informed approaches in interactions and service planning.
  • Safety & Risk Awareness – Demonstrates practices that promote resident, staff, and environmental safety.
  • Professional Conduct & Boundaries – Maintains therapeutic professionalism and appropriate boundaries.
Qualifications: Minimum Qualifications
  • Bachelor’s or Master’s degree in Social Work, Counseling, Psychology, Human Services, or a closely related field from an accredited institution.
  • Must meet the requirements of a QMHP in the state of Tennessee.
  • Valid driver’s license and acceptable driving record, if transportation or community coordination is required.
  • Ability to pass required background checks, drug screening, and any state-mandated employment clearances.
Preferred Qualifications
  • Experience working with children, adolescents, or individuals in behavioral health, residential treatment, or human services settings.
  • Knowledge of service planning, care coordination, and clinical documentation standards.
  • Familiarity with North Carolina behavioral health regulations and service systems.
Required Knowledge, Skills, and Abilities
  • Strong interpersonal and communication skills.
  • Ability to manage multiple priorities and maintain organized records.
  • Ability to work independently and as part of a multidisciplinary team.
  • Professional judgment, discretion, and ethical conduct.
  • Proficiency with electronic health records and standard office technology.
