Job summary

Trustpoint Rehabilitation Hospital of Lubbock is seeking a Case Manager to join our inpatient team in Lubbock, TX. The role involves collaborating with physicians and a multidisciplinary team to develop and monitor individualized care plans for both adult and pediatric patients in a premier rehabilitation setting.

Qualified candidates will have current licensure, at least one year of recent relevant experience in acute care, and a strong understanding of costs associated with treatment and

• Ability to understand treatment costs and financial considerations related to quality care.
• Demonstrates critical thinking and leadership in coordinating care and discharge planning.
• Maintains confidentiality and effective communication with the interdisciplinary team.

Responsibilities

• Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients.
• Monitors progress and resolves barriers to patient care.
• Actively involved in discharge planning and ensuring smooth transitions.

We are seeking a Case Manager to join our team of passionate patient caregivers!

Qualifications

Required Skills

• Current clinical licensure strongly preferred.

Responsibilities

Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process. Integrates the hospital’s mission and “Guiding Principles” into daily practice.
