• Monitor attendance and academic status.
• Identify and refer community resources.
• Conduct bi-weekly group sessions and monthly workshops.
• Assist with transitions back to school and workforce.
• Maintain individual case management files.
• Advocate for pregnant and parenting teens.
• Attend professional development courses and training.
