Qualifications

• Bachelor’s degree in a relevant field with social work focus and three years of experience.
• Master’s degree preferred but not required.
• Understanding of pregnancy, birth, parenting services and community resources.

Responsibilities

• Reach out to and engage new clients; schedule appointments with frequency (bi-weekly, at least monthly).
• Maintain a caseload of up to 25 active clients.
• Assess client strengths/needs to develop targeted service plans.
• Provide case management and counseling to support healthy pregnancies and parenting options.
• Encourage family and birth father involvement with client consent.
• Offer parenting education and coordinate community referrals and agency funds.
• Coordinate with external providers and deliver crisis intervention when needed.
• Represent the agency at community events and support program referrals.
• Maintain accurate client records for billing to funders.
• Participate in supervision, meetings, and ongoing training.
• Comply with HIPAA and Joint Commission standards; work collaboratively.
