Job summary

Community Partners in Strafford County seeks a Case Manager in Consolidated Services to be the primary contact for adults with Developmental Disabilities and/or Acquired Brain Injuries. You'"ll be the primary contact for individuals and families, ensuring smooth service delivery for consumers on your caseload.

You'"ll work with families to oversee and manage their programs and assist in supervising Family Managed Employees (FME’s) employed by Community Partners.

Qualifications

• Associates degree required; Bachelor's degree preferred.
• Minimum of two years’ experience in the human service industry preferred.
• Good math skills preferred.
• Excellent oral and written communication skills.
• Exceptional attention to detail.

The Role:

In this position you\'ll be the primary contact for individuals and families, ensuring smooth service delivery for consumers on your caseload. You\'ll work with families to oversee and manage their programs and assist in supervising Family Managed Employees (FME’s) employed by Community Partners.
