Job summary

Community Partners NH is seeking a Case Manager to organize, facilitate, and document service planning for individuals with developmental disabilities. You will collaborate with community resources to ensure high-quality service provision.

The ideal candidate has an Associate degree (Bachelor's preferred), strong communication skills, and the ability to multitask while coordinating with our team. This is a full-time on-site role in Dover, NH within a nonprofit organization offering a meaningful

Qualifications

  • Associate degree or equivalent required; Bachelor's Degree preferred.
  • Two-three years of relevant field experience preferred.
  • Excellent oral and written communication skills.
  • Ability to utilize technology provided to perform requirements of the job.
  • Hold a valid driver’s license and proof of vehicle liability coverage.
  • Must demonstrate excellent multitasking abilities.

The Role

As the Case Manager, you will be responsible for organizing, facilitating, and documenting the service planning for individuals with Developmental Disabilities as well as working collaboratively with other community resources to ensure the quality of service provision.
