Job summary

Community Partners in Strafford County is seeking a Team Leader of the Resource Center to oversee ServiceLink operations as part of the Information and Referral team. You will coordinate clinical care with clients, families, and partner providers, ensuring timely access to services.

Ideal candidates will have a high school diploma, 3+ years in social work or case management, strong communication skills, and the ability to use technology effectively.

Qualifications

High School Diploma or GED is required.
Minimum of three years of social work or case management experience.
Experience in customer relations, call management, information and referral.
Excellent oral and written communication skills.
Ability to utilize technology used for this job.
Valid driver’s license and vehicle liability coverage.

Responsibilities

Oversee ServiceLink operations as part of the I&R team.
Coordinate clinical care with clients, families, CP staff and other providers.
Lead and support the Resource Center Team to ensure timely assistance.

The Role:

As the Team Leader of the Resource Center, you will be overseeing ServiceLink operations as part of the Information and Referral (I&R) team, who are responsible for linking individuals who need assistance with appropriate service providers and/or supplies. You will also be coordinating clinical care with clients, families Community Partners staff and other community providers.

What we’re looking for:

High School Diploma or G.E.D.

Minimum of three years of social work or case management experience, preferably in the areas of aging, disabilities, community health, nursing home or hospital discharge planning

Experience in customer relations, call management, information and referral or related field that includes phone skills and preliminary assessment and triage ability of contacts

Excellent oral and written communication skills

Ability to utilize technology provided to perform requirements of the job

Hold a valid driver’s license and proof of vehicle liability coverage.
