Job summary

Professional Services Group, Inc. is hiring a Case Manager in Madison, WI. The role involves receiving referrals, performing needs assessments, and developing individualized service plans while managing a client caseload and coordinating community services.

Typical schedule is Monday–Friday 8:00am–5:00pm with occasional evenings. Local travel for home visits in Dane County; mileage reimbursement provided. Competitive benefits include PTO, health/dental/vision, life, disability, 401k, and EAP.

Qualifications

• Bachelor's degree in sociology, social work, psychology, or closely related is required.
• One year of professional social work experience is required.
• Must have reliable transportation, a valid driver’s license, and automobile insurance.

Key Responsibilities Of The Case Manager

• Complete in-person home visits to conduct assessments and maintain client contact.

SALARY: $26.00 per hour

LOCATION: Madison, WI. Local travel is required for client home visits throughout Dane County, with mileage reimbursement.

SCHEDULE: Monday-Friday 8:00am-5:00pm, with occasional evenings to accommodate families' schedules.
